Yes. It costs nothing and needs no card. You only pay if you later choose a plan, and even then the first 7 days are free.
Does it change anything in my Apple Ads?
No. The check-up only reads your account. AdSitter changes nothing unless you later start a plan and let it look after your campaigns.
